This has puzzled me for a while now. My site is closed and I've been working on it for some time. But I notice from time to time that the Users Online box will show that I have guests online. Sometimes 1 or 2, another time 12, and just now 19.

Am I missing something? If the site is closed, can people still be in there?

When a guest views your closed site, they'll see the closed message, however if you have enabled to collect stats when the site is closed, Composr will still track their page hits and thus they will appear on users online.
