Has the cms_f_custom_fields table somehow lost the auto_increment setting on the 'id' field? That's unusual, have you been hand editing or hand restoring that table?

Yeah - I was trying to restore specific tables from an older database. I think where I went wrong was when I failed to run the "alter table" commands on the table.

It's fixed now, thanks for pointing me in the right direction. :)
