I remember reporting this issue once before and it was closed. I'm reporting it again because it's happening again.

Might be a cache issue. When it does this, after a while it hyperlinks itself.

Think likely to be #2862.

If there's nothing to click to there's no link (i.e. no comment option or fields not shown), however after editing anything the persistent cache block cache often wasn't flushing correctly.
