Description	Move all install/uninstall code from modules and blocks into addon_registry hooks. Modules become 'controllers', handling page requests, but in no way responsible for defining model behaviour.

Note there are some TODO's in the code attached to this issue, relating to idealisation of where install code should be placed.

Decided that I like modules and blocks auto-installing and auto-upgrading. It isn't really wrong to do it like this, and it's good to not force every bit of installable code to run as a part of a formal addon.

However, addons will now be able to install/uninstall directly too.
